The United States National Vaccine Injury Compensation Act of 1986 established a government tracking system, VAERS (Vaccine Adverse Events Reporting System) that monitors reported vaccine injuries and deaths. The FDA estimates 90% of vaccine reactions are never reported to VAERS.

The following are excerpts from the VAERS database of 1570 events reported to the database of vaccine injury and death in New Jersey from June 2004 through May of 2008. This PDF is 550 pages long.
